We want you to benefit from the freedom to decide the best therapeutic options for your health needs. As a result we do not work with insurance companies. However, your insurance company may be willing to reimburse you for our visit. We would be considered "out of network" since our practice does not work with insurance. We are happy to provide you with an invoice to submit to your insurance for reimbursement. This will be your responsibility to follow up with insurance to receive reimbursement. Please note that Medicare beneficiaries are not entitled to any reimbursement for services we provide. Fees for services not covered by insurance and Medicare will be your sole responsibility.
We accept credit and debit cards (Visa, MasterCard and American Express). Payment is due at time of appointment with the nurse practitioner. We will be accepting HSA in the near future.
For weight loss, we ask that you complete some forms before you meet with us so that we can get to know you better. We highly encourage lab work, but at this time unless the medical director has determined there is a health concern, they are optional. We take a holistic approach and your likes and dislikes matter as much to us as your medical history.
For hormone optimization, we require labs prior to meeting with the medical provider so that a thorough review of results can be included with your visit. Your medical director will review your intake forms and determine which lab panels would be best for you prior to meeting and can send a lab order. We do not work with insurance so all lab costs are cash pay and costs will be reviewed with you and agreed upon prior to sending a lab order.
We offer a one hour wellness visit with the nurse practitioner for $150. No diagnostics are ordered without your consent as additional cost may be required pending your insurance provider approval. Wellness visits optional and are not the same as weight loss initial and follow up visits.
Labs may covered by health insurance. We have a relationship with both Quest and Labcorp and have negotiated reduced cash pay pricing for our clients. We require payment for labs prior to submitting the lab order. No additional diagnostics are ordered without your consent.
